Carex is partnering with a mission driven Wellness / SDoH startup industry partner to hire a State Partnerships Manager to solidify and expand the organization’s presence across the state of New York.
This is a high-impact, externally-facing role supporting healthcare access for low-income populations by forging strategic partnerships with Social Care Networks (SCNs), health providers, and community-based organizations.
This role requires a dynamic relationship-builder who thrives in both grassroots community settings and polished professional environments. The ideal candidate will have strong experience in B2B partnerships within healthcare, particularly with Medicaid populations, and will be based in New York with the ability to travel throughout the state.
What You’ll Do

Serve as the primary point of contact for all existing and new SCNs in New York

Attend SCN convenings and ensure internal teams are aligned on updates and changes

Identify and build relationships with uncontracted SCNs to expand the organization’s footprint

Develop a robust network of referral channels, focusing on FQHCs, healthcare providers, and community organizations

Source and qualify new partner leads through outbound efforts, community events, and digital platforms

Use CRM tools to manage the full partner pipeline from lead to active partner

Assess and monitor partners on referral capacity and performance

Drive collaboration across teams to improve communication, data sharing, and value delivery to partners

Align with Member Engagement Coordinators to optimize their capacity and workflow
-
Deliver consistent reporting on key performance indicators and partnership progress

Provide strategic insights from the field to inform product and service development
What You’ll Bring
-
5+ years of experience in partnership development, account management, or B2B sales, ideally in a fast-growing environment

Strong relationship-building and partnership management skills, both in-person and remotely
-
Familiarity with the Medicaid ecosystem, particularly 1115 waivers and community health structures
-
Proven success in healthcare-related partnerships or B2B sales

Deep understanding of the landscape of healthcare and community orgs that serve Medicaid members
-
Confident public speaker with excellent client-facing and presentation abilities

Proactive and resourceful, equally effective in grassroots outreach and executive-level engagements
-
Comfortable with CRM tools and data-driven performance tracking

Strategic mindset with the ability to translate business goals into field-level action
-
Results-oriented and open to stepping outside your comfort zone to drive outcomes

Passion for health equity and improving quality of life through innovative food-as-medicine models

Comfortable working in distributed teams and excited about contributing to a high-growth startup environment
