Carex is partnering with a mission driven Wellness / SDoH startup industry partner to hire a State Account Manager based in New York.
Summary
The Account Manager will drive relationship management and growth initiatives across the state of New York, serving as the primary point of contact for Social Care Networks (SCNs) and community-based referral partners. This role will focus on strengthening existing partnerships, earning new SCN contracts, and establishing a strong pipeline of organizations that refer eligible members. Reporting to the VP of Marketing & Member Operations, the Account Manager must be based in New York and able to travel regularly throughout the state.
What You’ll Do

Act as the lead relationship manager for all contracted and prospective SCNs in New York

Represent the company at SCN convenings, keeping internal teams informed of updates and changes

Proactively identify and engage SCNs not currently under contract

Build a pipeline of member-referring organizations, including FQHCs, healthcare providers, and community-based organizations

Qualify and engage partner leads through outbound outreach (phone, email, social media), in-person meetings, and events

Use CRM tools to track and manage partner pipelines from lead to active referral source

Evaluate referral partners for capacity and performance and identify ways to increase engagement

Collaborate cross-functionally to improve communication and data sharing with partners

Align with Member Engagement Coordinators to ensure efficient handoff and support of referred members

Report regularly on progress toward individual and company-wide goals and OKRs

Share real-time market insights with internal teams to support product and service refinement
What You’ll Bring
-
Minimum of 5 years’ experience in Account Management, Partnership Development, or B2B Sales in a growth-stage organization

Strong interpersonal and relationship-building skills, both in-person and virtually

Must reside in New York and be able to travel frequently throughout the state

Knowledge of the Medicaid landscape, including 1115 waivers and SCN structures

Demonstrated success working with healthcare providers and community-based organizations

Understanding of Medicaid-serving populations and their community support ecosystems

Strong communication, presentation, and CRM skills

Ability to bridge strategy with on-the-ground implementation

Results-driven, self-motivated, and comfortable owning performance metrics

Passion for using food and social interventions to improve the lives of low-income individuals

Comfortable working in a distributed, remote-first environment

Thrives in a fast-paced, scaling company environment
Compensation

Base salary range: $80,000–$120,000
